Sliding Fee Discount Program

LifeTree Counseling Center offers a Sliding Fee Discount Program to help make counseling more affordable for individuals and families who may be unable to pay the full cost of services. Eligibility is based primarily on household income and size while also considering certain exceptional expenses and financial resources that may affect a household's ability to pay.

How do I qualify?

To qualify for the sliding fee scale, you must show proof of gross annual income for all household members. Gross annual income includes taxable income, non-taxable income, disability benefits, and public assistance.

Acceptable Income Verification:

  • Recent Federal tax return

  • IRS form, W-2, or 1099

  • Two recent pay stubs

  • SSA or VA benefit award letter

  • Letter from employer

What are “exceptional expenses”?

We understand that life will often throw curveballs with unexpected expenses that can cause hardship for a family. At LifeTree, we will consider these expenses as we process your application to see what type of discount you would qualify for. Documented exceptional expenses may be considered during the review process and may affect the discount level for which an applicant qualifies. Documentation will be required to justify these expenses as a deduction.

How to Apply

Sliding Fee Discount Program applications can be requested at our office or accessed on our website. Once completed, the application and required income verification documents should be submitted to the office manager in person or via email at info@lifetreecounseling.org. LifeTree Counseling Center will make every effort to review complete applications within 2 business days and notify applicants of the decision within 3 business days. New clients will then be scheduled or waitlisted if there are no immediate openings at the time of application.

Discounted services would become effective on the date of application approval for previously established clients.
