Your Rights as a Health Care Consumer

In accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105, LifeTree Counseling Center provides the following information on how to access your records and how to contact appropriate oversight agencies.

Requesting Your Health Care Records

Clients and former clients may request copies of their health care records using one of the following methods:

  • Email (preferred): info@lifetreecounseling.org

  • Mail or In Person: LifeTree Counseling Center, 201 Enterprise Ave., Suite 300
    League City, TX 77573

To request records, submit a written request that includes:

  • Your full name and date of birth

  • Contact information

  • Specific records requested

  • Preferred delivery method (secure email, mail, or in-person pick-up)

  • Signature and date

Requests made on behalf of another person require documentation of legal authority (e.g., guardianship or medical power of attorney). Identity verification may be required.
